This range of two to six days encompasses the various stages of embryo transfer and embryogenesis. These stages are cleavage stage and blastocyst stage. Cleavage stage comes after the two to four days of incubation. At this stage embryos are there to be transferred into the womb. This stage allows cumulative clinical pregnancy rate higher. Next is the blastocyst stage that comes after five to seven days culture. Blastocyst stage certainly improves the pregnancy chances but at the same time there is great reduction in remaining number of embryos for cryopreservation or embryo transfer.

How embryo culture is done

Embryo can be cultured in two mediums- one is autologous endometrial coculture and other is artificial culture medium. Blastocyst transfer- Dr Sarita Teotia

Blastocysts are the incubated embryos after five or six day’s growth. Best quality embryos are selected and separated from the rest at this stage only. These separated high quality embryos are then cultured in the incubator. Out of these incubated embryos which were best amongst a lot, one or two are selected to be implanted in womb. These selected incubated embryos are blastocysts.

Pregnancy shall be successful only if these blastocysts stick themselves to the wall of womb after implantation. And this is the reason for implanting more than one blastocyst. So that atleast one of them attaches itself to the walls of womb.

Blastocyst transfer improves the IVF success rate. This works well with younger women with reasonable pregnancy probability and good quality embryos. Because, blastocyst transfer shall be successful only if embryos are of good quality. 

This process of blastocyst transfer helps achieving success in pregnancy because blastocysts are more robust and strengthened embryos of six days incubation than that of normal embryos of two or three day’s growth.

But as nothing in this world is infallible, so is the process of blastocyst. There are few setbacks and risks associated with it. Firstly, due to poor motility, sometimes embryos don’t show any development after day two when it reaches four cell stage. So it is not possible to incubate and select blastocyst. Next is that there are chances of as number of multiple births as number of blastocysts are transferred.

Surrogacy refers to the arrangement of carrying a pregnancy for somebody else. A woman agrees to carry pregnancy and deliver the child. If the surrogate is getting paid anything beyond medical reimbursements, it is known as commercial surrogacy otherwise altruistic surrogacy. When it is not possible for the woman to carry pregnancy, she doesn’t want to be pregnant but wants child or risk to mother’s health due to pregnancy is unacceptably high, then couple may undergo surrogacy arrangement.

Surrogacy is suitable to the female with medical conditions that may not be favorable for pregnancy. These conditions can be infantile uterus, surgically removed uterus, damaged uterus, abnormal uterus, recurrent pregnancy losses, repeated IVF failures, etc.

Surrogacy can be classified in two main types- traditional or partial or straight surrogacy and gestational or full or host surrogacy. The main difference between the two is the genetic participation of surrogate. In traditional surrogacy, impregnation may be done artificially or naturally but the baby will relate genetically to the surrogate whereas in gestational surrogacy, embryo resulting from IVF is implanted into surrogate and the baby isn’t genetically related to her.


These two types can be further subdivided as:
